Part 2: The Ramp
Building Exposure Before Competition
In Part 1, we discussed the importance of building a strong foundation.
Strength, power, fitness, tissue capacity, and movement quality provide the physical foundation required for sport. But having the capacity to perform and being prepared to compete are not necessarily the same thing.
This is where the second phase of the process begins: the ramp.
Capacity Is Not Competition
Having the physical traits for sport doesn’t mean an athlete is ready for competition. Success requires more than just strength, symmetry, or the bike conditioning they’ve been doing twice a week.
Sport isn't a maximal strength test. It isn't a force plate assessment or a hop test.
The gap is an exposure problem.
Players are repeatedly exposed to accelerations, decelerations, jumps, landings, changes of direction, physical contact, cognitive demands, emotional stress, and decision-making requirements. The challenge isn't whether athletes can perform these actions once. The challenge is whether they can tolerate them repeatedly.
Building the Ramp
That's why I often think about return to sport (and performance) as a ramp rather than a finish line.
The goal isn't simply to pass return to sport testing and be ready for a return-to-play date. It’s critical to progressively expose athletes to increasing levels of volume and intensity so that the demands of competition no longer become a shock to the system.
Too steep of a ramp and the system becomes overwhelmed. Too shallow of a ramp and they may arrive underprepared. The art is finding the right progression and figuring out how to make this happen in your environment.
Over time, athletes need to be exposed to sport movement demands, higher running volumes, more jumping and landing exposures, increased court/field time, decision-making requirements, and ultimately sport-specific intensity. The exact progression will vary depending on the athlete, injury, and sport, but the principle remains the same.
The Athlete Response
Every session becomes an opportunity to collect information.
How did the athlete tolerate the workload?
How did they recover?
Did movement quality change under fatigue?
How did they respond psychologically?
The answers to those questions help determine whether the progression should continue, pause, or change direction.
Preparing for Reality
What makes this particularly challenging in basketball is that competition itself isn't linear.
The demands fluctuate constantly. A player may return to limited minutes, then find themselves playing three games in six days. They may travel across the country, play extended minutes because of foul trouble or injuries elsewhere on the roster, or be asked to take on a different role than expected.
The game doesn't always provide the perfectly controlled progression we would like. Which is why rehabilitation has to prepare athletes for those realities.
From Control to Chaos
My perspective on this concept has been heavily influenced by the work of Matt Taberner and his Control to Chaos Continuum.
The framework provides a practical way to think about progressing athletes from highly controlled rehabilitation environments toward the unpredictable demands of sport. Athletes begin in environments where variables can be tightly managed. Spaces are controlled, movement patterns are predictable, and decision-making demands are intentionally limited.
As rehabilitation progresses, those constraints gradually loosen. Speed increases, movement variability expands, and athletes are asked to solve increasingly complex movement problems that begin to resemble the realities of competition.
Eventually, the athlete is exposed to the very thing we are trying to prepare them for: chaos.
And basketball is chaos.
No two possessions are identical. No two cuts are identical. No two landings are identical. Players must constantly perceive, adapt, react, and solve problems in real time.
Building the Ramp Within Real Environments
One of the challenges with progressive exposure is that it rarely occurs in ideal conditions.
Most rehabilitation frameworks are presented as if clinicians have complete control over the athlete's environment. In reality, rehabilitation occurs within teams, schedules, seasons, staffing structures, and competitive calendars that don't always cooperate.
A player returning during the offseason presents a very different challenge than a player returning in the middle of conference play.
During the offseason, there is often greater flexibility to build exposure gradually. Workloads can be manipulated more freely, skill sessions can be structured around rehabilitation goals, and live basketball activities can be introduced in a progressive manner.
In-season returns are often far more complicated.
Practice opportunities may be limited. Team priorities shift toward game preparation and recovery. Coaches are focused on winning games and available personnel, not necessarily creating ideal rehabilitation environments.
One of the biggest challenges in these situations is creating appropriate basketball exposures without immediately exposing the athlete to unrestricted competition.
Live basketball sits near the top of the intensity spectrum. The unpredictable movements, reactions, contact, and decision-making demands are difficult to fully replicate in controlled rehabilitation settings.
Creating live opportunities isn't easy.
During the season, asking healthy players to participate in rehabilitation-focused activities may not be realistic. Coaches may be hesitant to increase workloads on key rotation players. Team schedules may leave little time for additional court sessions. In some situations, there simply may not be enough available players to create meaningful live scenarios.
As a result, clinicians often have to become creative.
Small-sided games.
Modified constraints.
Positional drills.
Coach-guided skill sessions.
Controlled competitive scenarios.
Each can provide valuable exposures while allowing the rehabilitation team to manage workload and complexity.
The goal isn't necessarily to recreate competition perfectly.
The goal is to expose athletes to enough of the physical, cognitive, and psychological demands of competition that the transition back to full participation feels familiar rather than overwhelming.
This is where collaboration becomes particularly important.
The best ramps I've been around rarely belong to a single clinician or department. They are built through ongoing communication between medical staff, performance staff, coaches, and athletes. Everyone understands the current objective, the next step in the progression, and the demands that still need to be addressed before competition.
Because ultimately, whether you’re based in a clinic or the training room…the sporting environment is part of the rehabilitation process.
And the better we understand the environment, the better we can build the ramp.
Performance Is Developed Through Exposure
The goal of the ramp is not simply to prepare athletes for movement. The goal is to prepare them for uncertainty.
Because performance isn't built through protection.
It’s developed through exposure.
In Part 3, we'll discuss what I believe is one of the most overlooked aspects of rehabilitation: using skill development as the mechanism that reconnects athletes with the sport itself and why collaboration with coaches may be one of the most powerful tools we have for restoring performance.
